Integrated Medical Processes (IMP), a joint consulting venture between The Kinetix Group (TKG) and the Consortium for Southeastern Hypertension Control (COSEHC), has garnered outstanding expertise in operationalizing value-based care. TKG is proud to provide consulting services to Blue Cross Blue Shield of Louisiana (BCBSLA) in successfully implementing Quality Blue Primary Care. This engagement boasts particularly fantastic results in this area of healthcare innovation, yielding cost savings of $27 per-member per-month (PMPM) through reduced hospitalizations and increased primary services.
What is the secret formula to attain such remarkable change? We are all familiar with the three pillars of the Triple Aim: utilizing a population health management approach, improving patient experience of care, and reducing the per capita cost of healthcare. Yet in attempting to achieve this trifecta, health systems often fail to recognize the importance of engaging the care delivery team. Consequently, TKG has made it our responsibility to promote a Quadruple Aim with our partners, which includes the empowerment of the care team as an essential fourth pillar to the original IHI model.
TKG uses a simple solution that is highly effective in re-engaging the care team: explain why the value-based care changes are occurring. When we take the time to discuss value-based care and its benefits to patients, care delivery teams are significantly more receptive to change. Not only have we observed increased adherence to processes with an engaged care team, but we have also witnessed more effective activation of patients. For example, a unique facet of the program to help maintain care team engagement is the introduction of a Quality Navigator into each practice, who assists in case management of chronic care patients and acts as a liaison between the provider and BCBSLA.
